The Minto-Mapleton Family Health Team (FHT) consists of a group of
health professionals working together with 8 primary care physicians
who have a combined patient roster of over 11,000 patients. While the
corporate headquarters for the FHT are located in Drayton, a team of
Registered Nurses, Social Workers, Dietitian, Health Promoter and part-time
Pharmacist, are available to patients in the Clifford, Palmerston, Harriston,
Moorefield and Drayton communities.
Our Mission
The mission of the FHT is to provide patient-focused, high quality
care that will help patients manage and prevent disease while improving
overall health and quality of life. The Team is 100% funded by the Ontario
Ministry of Health and Long-Term Care.
