The Minto-Mapleton Family Health Team (MMFHT) is an interdisciplinary team of healthcare professionals who provide primary care to over 15,000 patients. We have been funded by the Ministry of Health since 2006 and are proud partners of the Guelph Wellington Ontario Heath Team since 2021.
