What is an Ontario Health Team (OHT)?
In 2019, the Ontario Government introduced a new model of integrated health care called Ontario Health Teams (OHTs). OHTs consist of groups of care providers, community organizations, patients, and caregivers who work together to design an improved, more coordinated system of care in their communities.
The Minto Mapleton Family Health Team is a core member of the Guelph-Wellington OHT. MMFHT leadership and a board representative participate on the Guelph Wellington OHT's Steering Committee and Governance Council. Several MMFHT staff participate on one of the many Working Groups.
