Dr. Deacon is the Medical Lead for the MMFHT. His main role is overseeing the clinical activities of the MMFHT, and creating a collaborative team environment for its employees. Dr. Deacon will work closely with the MMFHT Executive Director to ensure appropriate clinical policies and procedures, medical directives are in place. He will engage with team members to deliver patient focused, quality care in all programs and services offered. The Medical Lead works collaboratively with the Executive Director to communicate information about clinical activity and programs to the Board.
The Chronic Disease Management Nurse provides a pro-active, patient-centered approach that addresses chronic disease early in the disease cycle. The focus is to delay progression and reduce potential health complications.

The Clinical Nurses include registered nurse Leah at our Clifford site, and registered practical nurse Brittany at our Harriston site. The Nurse Practitioners (NP) independently diagnose, treat and manage acute and chronic illness across the lifespan. They work collaboratively with the physicians and allied health professionals at the MMFHT to support your needs.
Physician Assistants (PAs) are highly skilled health care professionals who function as an extension of physicians thereby increasing your access to care. They diagnose and treat acute and chronic health conditions and provide preventative care to all ages.
With the help of our TM Nurses, telemedicine allows you to have appointments with specialists in your home community. The TM Nurses coordinate appointments for you and make all the arrangements necessary for you to experience a productive, welcoming visit. Often, they attend the appointment with you and assist with communication before, during and after the consultation.
The Outreach Worker is available to help you reach out and find the resources you need, when you need them. She will help you get connected to community agencies, to navigate social programs, and support you when family or friends can't help. The Executive Director is responsible for providing leadership in the operation of MMFHT offices and the recruitment of qualified employees. She is accountable to the Board of Directors to advise on matters of policy, health, human resources, and financial matters. The Executive Director is responsible for the development of an annual business plan and budget and is expected to regularly interface with the Ontario Health's MMFHT Consultant. She reports directly to the MMFHT's full Board of Directors at bi-monthly board meetings. MMFHT employees look to the Executive Director for leadership, problem solving and support.
The Medical Receptionist in the Drayton office provides receptionist and administrative support to MMFHT Team members and the Executive Director . Linda is a friendly face and voice that greets our patients and the public in Drayton. She is the primary contact for booking appointments with Team members in Drayton and Palmerston.
The Medical Receptionist at our Harriston site works closely with the MMFHT Nurse Practitioner and Clinical Nurse. Kawkab organizes and coordinates activities of the Harriston site in collaboration with the onsite Clinical Nurse and Nurse Practitioner. The Medical Receptionist performs tasks such as reception, appointment booking and making arrangement for referrals requested by the MMFHT Nurse Practitioner.
The Medical Receptionist at our Clifford site works closely with the MMFHT Nurse Practitioner and Clinical Nurse. Sherry also provides support to visiting physicians and MMFHT Team members when they visit the Clifford site. She organizes and coordinates activities of the Clifford site in collaboration with the onsite Clinical Nurse and Nurse Practitioner. The Medical Receptionist performs tasks such as reception, appointment booking and making arrangement for referrals requested by the MMFHT Nurse Practitioner and Physicians.
