We are interested in hearing from you about a recent visit with your physician, nurse practitioner or any of the Family Health Team members.  Please take 5 minutes to complete the survey through the link below. All information provided is confidential and anonymous.

If you have a specific concern about a provider, it is best to discuss with the provider directly.  If you have a general comment or concern about our services or facilities, you can provide feedback to the Executive Director of the Family Health Team:
