Diabetes & Pre-Diabetes

The MMFHT, in collaboration with the Palmerston District Hospital Diabetes Education Centre, is seeing patients with Type 2 Diabetes.  The program has been designed to provide you care in accordance with the Diabetes Canada Guidelines.  Your appointments are with one of our Certified Diabetes Educators (Registered Dietitians and a Registered Nurse).  Visits are hosted in Drayton, Palmerston, Clifford, or virtually.

We can also offer individual counselling for pre-diabetes to assist you with positive lifestyle changes that will help you manage your health.
